Can I book an appointment through the Entitlement Unit?
No. The Entitlement Unit does not handle appointments. Appointments must be booked with Primary HealthCare (Public Health Centers) on 21231231 / 21222444 or [email protected], or with a private clinic at your own expense.
IMPORTANT – Please read carefully:
- Only single-vision lenses (distance and/or near) are covered.
- Approval will be for a one-year period (twelve [12] months).
- Applications for free spectacles can only be made after 12 months from the last prescription’s date e.g. if your last prescription was dated 1/12/22, your next prescription should be dated 2/12/23 or later.
- No refunds of extra expenses on ordered spectacles e.g. different frames other than what is offered, high-index (thinner) lenses, etc.
- Change of lenses are not covered: kindly confirm beforehand with your optometrist your prescription is correct.
- Repairs are not covered. Clients are fully responsible for the spectacles’ aftercare and use.
What to do
- Visit an optometrist for a prescription.
- The doctor’s prescription must be original and written in full on the form entitled ‘PRESCRIPTION FOR FREE SPECTACLES’ of the Department of Health (see picture). Both your prescription’s white and pink carbon copies are required.
If 2 pairs (distance and near) are needed, this must be indicated on the same prescription.

- A copy of your valid Pink Form obtained from the Social Security Dept – Contact details here JEW
- A copy of your official, valid Schedule V document (all pages), obtained from the POYC (Pharmacy of Your Choice). Contact details here.
- Your home address (where a reply will be received) and contact number
Expired or incomplete documents are not accepted.
Post the original signed prescription (both white and pink carbon copies) u copies of your Pink Form/Schedule V document to: Entitlement Unit, Ground Floor, Ex-Outpatients Block, St. Luke’s Hospital, G’Mangia Hill, G’Mangia
If you are entitled, you will receive a document by post to inform you where to take the prescription and documents to have your spectacles done.
