Terms and Conditions
- Only single vision lenses (distance and/or near vision) are covered under the scheme.
- Varifocal and bifocal lenses are not covered.
- New applications may only be submitted 12 months after your last prescription date.
Once spectacles are provided:
- Changes of lenses are not covered. Ensure your prescription is correct before you submit your application.
- Repairs are not covered. Clients are responsible for the care and use of their spectacles.

Applicants may also visit a private optometrist at their own expense.
What to do
1. Visit an optometrist for an eye examination and inform them you wish to apply for the Free Spectacles Scheme.
2. The optometrist provides the official “PRESCRIPTION FOR FREE SPECTACLES” form required for the application. This is only available from the optometrist.
3. The prescription must:
- Be original.
- Be completed in full on the official “PRESCRIPTION FOR FREE SPECTACLES” form issued by the Department of Health.
- Be submitted with both its white and pink carbon copies.
- Show your correct postal address, as all correspondence will be sent to that address.
4. Include 1 of the following:
Either
- A copy of your valid Pink Form issued by the Social Security Department (Freephone 153)
Or
- A copy of your valid Schedule V (yellow form) showing diabetic medication, issued by POYC (Tel: 22481800 / [email protected])
Expired or incomplete documents are not accepted.
5. Submit your application by post.

- Your Original prescription (White and pink carbon copies)
- Copy of your Pink Form or your Schedule V

6. What happens next?
If you are eligible, the Entitlement Unit will later send you a document by post explaining where to present your approved prescription so you can order your spectacles.
